Dear patient, we are looking forward to meeting you and working with you on making you healthier! Please kindly take a note of the following policies as there are some important differences from most conventional medical practices you've probably worked with before. 


POLICIES

  •  We are a consultative practice. We are not your conventional primary care physician’s office. In fact, we require you to maintain a relationship with a primary care physician even though in many instances we can provide conventional services (such as prescribing an antibiotic in a minor illness i.e. sinusitis or otitis, etc.) that ordinarily a primary care physician provides, in addition to our specialty services
  • We are not in network with any insurance plan so we do not fulfill an insurance company requirements for an in network primary care physician
  • We do not have a 24 hour answering service and do not take calls on weekends. If you have a medical emergency or need to be seen urgently, you would need to either dial 911 or go to a nearest ER or to an urgent care center
  • We are not available for the same day emergency appointments (in the majority of cases). Our availability is usually only during normal business hours and can be limited at any given time.

 

BILLING

We do not bill insurance companies. You will be billed directly at the time of the appointment and given a receipt (a super bill) upon completion of an appointment, which you may submit to your insurance carrier for reimbursement. Many conventional laboratory tests will be covered by your insurance carrier as well as some of the functional medicine tests may be. However you should always check the coverage with your insurance plan prior to undergoing any testing.

 

APPOINTMENT SCHEDULING

  • Appointments must be scheduled in advance during Healthyon’s available business hours. Scheduling will be on a first come basis, based on availability. In case you need some special timing for your appointment, we will make every effort to accommodate your wish but cannot guarantee it. There is a 24 hours cancellation policy. Canceling within 24 hours will result in a $100 charge to the credit card on file.
  • Every initial appointment must be in person. Follow ups are available either in person or via the phone. Initial appointments last 75-90 minutes. Follow ups are either 60 or 30 minutes long (but they can be both longer and shorter depending on your need). The first follow up is always at least 60 min.
  • No shows: if you do not show up to an in person, phone, video/telemedicine          appointment without notifying the specific person you have an appointment with, you will be charged $150 to the card on file.
  • Prior to your first scheduled visit you will be required to provide medical,  personal and overall health information and sign certain authorizations, releases and other documents regarding the care that will be provided to you at Healthyon Institute. You will not be able to access our services until we have all the necessary paperwork as we require. Instructions for completion of the paperwork will be provided separately.
  • Please remember to bring to your visit pertinent medical records, as well as all your medications and supplements in their original packaging.
  • Email. Emailing will be available only through secure patient portal. Please be advised that all the questions that are determined to be too complicated or too involved to be handled over the secure patient portal either by your Helathyon Institute physician or any of the Healthyon Institute staff, will require a formal appointment either in person or over the phone.

 

LABORATORY TESTING

  • In most instances they will be billed directly to your insurance company by the lab.
  • Some of the functional medicine tests may not be covered by your insurance and you will be charged a fee for a particular test by the lab that performs it. The lab fee is not under Healthyon Institute's control, and neither is the decision of the insurance company on whether to cover the lab work. The lab charges can be significant - please take this into account. You are advised to check with your insurance company on the available coverage.
  • The testing protocol will be personalized to you based on your initial and subsequent evaluations by your Healthyon Institute physician.
  • At home specialty test kits: if a lab test is either dispensed or sent to you by Healthyon, you will be charged for it on the same day. This is not included in the visit charge or a membership plan.
  • You are NEVER under an obligation to purchase any specialty, functional medicine tests.
  • We will make every effort to inform you about the cost of testing but encourage you always check with your insurance carrier.
  • If your insurance plan requires you to undergo testing only at a specific lab facility (such as Quest or a hospital lab), you must inform us about it prior to us referring you for testing.
  • If your lab results indicate that you need further testing, you will have an option either to continue to work with your Healthyon Institute provider or to go back to your primary care physician. Sometimes a consultation with a relevant specialist is necessary.
